The inscription, rendered with a fiber-tip marker, is placed in the upper right corner and reads To Jack / Elvis Presley; a facsimile autograph is printed below on the King's blue blazer while nine Elvis albums are advertised on the back of the sheet; a notarized Letter of Authenticity is included and is signed by James Spence of JSA Authentication Ft. Lauderdale, Florida.

Here is the consignor's best recollection of getting Elvis’ signature: "It was my first airplane flight ever, as my parents and I flew from Virginia to Hawaii. My family was there for a convention of florists at the Ilikai Hotel on Waikiki Beach. Early 60’s, and I would have been 11,12, 13 maybe, at that time. I heard Elvis was next door and went over (it could have been a part of youth activities organized by the convention staff, but I’m not sure,) to the Hilton Hawaiian Village which was pretty much next door to the Ilikai Hotel where my family was staying. I joined a long line and Elvis asked my name, personally signed photo, and sent me on my way with a smile. My mom kept it for years after I went off to college and career. She died in 2009 and I found it, again, among her files. I’ve had it locked away ever since."
